THE BIG CONVERSATION
It's not just fat loss. It's a change in how the skin repairs itself.
Most people talk about weight loss medication as simple volume loss. The biology underneath is more interesting. There is a thin layer of fat within the skin itself, the DWAT, which feeds the cells that make collagen. Early research suggests it becomes less active with rapid weight loss. It is a fascinating clue rather than proof, but it explains why some patients' skin looks thinner and more crepey than their weight loss alone would suggest.
So my advice now starts much earlier, ideally before someone begins. Pace matters, because faster isn't better for your face. Protecting muscle with protein and strength training is non negotiable. Gentle collagen stimulation can begin early, so we protect the skin rather than rescue it later. What I won't do is volumise or lift while the weight is still moving.
And it has to be 360. The medication works on the metabolism, but the mind needs support too, because body image often lags behind the body.
The medication is a tool, not the plan.

IN MY OWN WORDS
What I took to the panel.
01
A thread lifts. It doesn't fill.
Ageing and weight loss do three things: we deflate, lose skin quality and descend. A thread only treats the third. When results keep shrinking, the honest answer is a surgeon.
02
From filling to regenerating.
Patients want to look like themselves, rested and well. I use far less filler than ten years ago. Most of my work now stimulates the body's own collagen. Foundations before decorations.
03
Test, not guess.
The skin is a mirror of internal health. Inflammation, hormones and blood sugar shape every result. Optimal, not normal. And promising is not the same word as proven.
04
A good no.
Warm, clear and always with an alternative. Sometimes I show patients their photos side by side so they can see how far they have come. My reputation is built on the treatments I didn't do.
